Rates

Therapy:

Intake (initial appointment) – $200 for 60 – 90 minute session
60-minute individual/Couples session – $180 per 60 minute session

**We offer a sliding scale fee opportunity.**

Interns are seen at a fee rate of $30/hr. This fee is applied to offset their administrative and supervision costs.

4LCC is in-network with the following North Carolina Insurance:

If you have an insurance that we do not accept, we are capable of providing a superbill. Please contact your insurance company to determine if you have out-of-network services, which would allow us to provide this service.

Be advised: Not all services are deemed medically necessary and therefore not covered by insurance.

At this time, we are not able to take insurance in GA.

Payment

We DO NOT accept cash payment. We DO accept all major credit cards.*

No Surprise Act

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.

Under the No Surprise Act, providers need to give patients who do not have insurance or who are not using insurance an estimate of the bill for medical items and services.

• You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.

• Your will receive a Good Faith Estimate in writing at least 1 business day before your service. You can also ask your provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service.

• If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

• Make sure to save a copy or picture of your Good Faith Estimate. For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises or call (910) 356-9213.
